Following is a questionnaire to help us pick a meeting time 
that will work the best for most people.

If you have any desire to attend meetings, PLEASE fill out and
return this questionnaire to:   bruce@armintl.com   Thank you!

This questionnaire assumes that most people work or go to school
during the day Monday-Friday, and this is generally a bad time.  
If this was a bad assumption on my part, please let me know in 
the comments section.

Please rate ALL the following date/times on a scale of 1 to 5.
1 being the LEAST desirable time, 5 being the MOST desirable.
Any unanswered lines will be given a score of "1".

  5 = could attend almost all the meetings at this time.
  4 = could attend most of the meetings at this time.
  3 = could attend about half of the meetings at this time.
  2 = could attend a few of the meetings at this time.
  1 = could NOT attend most or any meetings at this time.

Also include your ideal time to START meetings for each line.
  (i.e.   "3:00PM"  or  "After 5:30PM"  or  "6:00PM - 7:00PM")
