COM2 & COM4 both share the same IRQ3. Having both of those COM
ports active could create a conflict. Same for COM1 & COM3 (IRQ4).

I personally like to debug modems in minicom.
See if you can get minicom running and connect directly to the modem
and issue some AT commands. Also try dialing MANUALLY from minicom
(do not use it dialer menu). If your ISP's modem number is: 555-1234
then issue the command, and see if your modem dials:
